Change of Address
Change of Address?? How to edit/update in Final Forms.
When you have an address change at any time during the school year whether it’s inside OR outside of the school district, as a parent/guardian you’re required to update this information in Final Forms which is our online enrollment module. We will accept phone calls with questions however there is a process that MUST be completed before the information gets to our transportation department.
- Login to Final Forms and edit the Contact Information Form with the “NEW” physical address for the student AND each adult (if applicable) at the bottom sign your name and click SUBMIT FORM
- Move to the Transportation Information Form and edit and update as needed at the bottom sign your name and click SUBMIT FORM
- A notarized Residency Affidavit must be completed fully, along with 2 proofs of the new address and it must be uploaded to Final Forms. The Residency Affidavit is below. Clicking on it will open the PDF that you can print. This information is proof you live where you state you’re living.
Once this is complete, administration receives an alert of the change and moves it to our student information system…then it moves to our transportation module. (This will take 24 hours) As you can see this is a process and it’s not immediate. So PLEASE remember to gather your information, update and upload to Final Forms, then you’ll receive an email from transportation as to the new busing information for your student.
If all these steps are not completed, we don’t move the information.
